Take the survey – help us improve children’s health
At Children’s Hospital of Richmond at VCU, we nurture the champion in every child. To do this, we need your help. We are assessing the health needs in the communities we serve. All families in our community are invited to participate in a brief survey – you do not have to be a CHoR family or use CHoR services to participate.
Why your opinion matters
Your answers will help us understand the biggest health challenges children face. This will help us create better programs and services to support their health.
Who can take the survey
Parents, caregivers, health care workers, teachers and anyone who cares about children’s health are welcome to take the survey.
Your privacy
The survey is confidential, so you will not be identified in the survey results. This survey is being administered on behalf of CHoR by Community Health Solutions, Inc. The survey information will be analyzed with other data for a comprehensive community health needs assessment of our area.
Time needed
The survey will take about 10 minutes. We are very thankful for your time and honest answers.
How we will use your feedback
Your input will help CHoR and our partners better meet the needs of the children we serve. Together we can make our community healthier and stronger.
